I hereby request a copy of the public portion (information that ordinarily appears on the first page) of an offense report from the University of Texas Police at Houston. I understand that all requests will be processed within ten (10) normal business days, not including the day of receipt.
The public portion of a police report contains all pertinent information necessary to file insurance claims, notify employers or property owners of the incident or for your personal files. For further information, contact the UT Police Records Office at:

Complaints or suggestions received by the Chief of Police will be screened to determine if it is a complaint against an employee (violation of rules and regulations) or a recommendation for improvement of the department at large (process improvement). Verbal or anonymous complaints are also investigated but are more difficult to process.
MD Anderson workforce members and students may complete the following form to request access to the MD Anderson Bike Centers. The Pickens Academic Tower, the Mid-Campus Building, or Zayed buildings all feature showers and lockers for bike riders.
Please note: For MD Anderson Workforce Members and Students Only. MD Anderson workforce members and students must complete bike registration online at: www.project529.com before requesting access to MD Anderson Bike Centers. Incomplete bike storage access requests may result in your request not being processed. UT Police uses the project529.com database to locate missing bicycles.UTHealth Medical School Building occupants have automatic access to the Medical School bike barn. At this moment, there is no other bike storage for UTHealth Houston students, employees, faculty and staff. Even so, UTHealth Houston students, faculty and staff who commute to campus are encouraged to complete bike registration at: www.project529.com.

Bike Registration FormThe purpose of this public portal is to receive comments regarding an agency's compliance with CALEA standards, engagement in the service community, delivery of public safety services, and overall candidacy for accredited status. These comments can be in the form of commendations or concerns. The overall intent of the accreditation process is to provide the participating agency with information to support continuous improvement, as well as foster the pursuit of professional excellence.
IMPORTANT: CALEA is not an investigatory body and subsequently the public portal should not be used to submit information for such purposes. Additionally, there will be no response other than acknowledgement to submissions; however, the information will be considered in context to its relevancy to compliance with standards and the tenets of CALEA® Accreditation.
